Gynecomastia

As plenty of concerns are focused on the body shape nowadays, people try to obtain the muscular and well-balanced figure. Especially for the man, pectoralis and rectus abdominis muscle are indispensable to this attractive figure. However, westernized diet with lack of appropriate exercise makes increasing obese features. Morbid fatty deposition around the whole body and disrupted body figure are now easily seen elsewhere. In general, male with moderate degree of obesity tends to have woman-like chest because of the fatty accumulation around the breast. Gynecomastia implies that male has female-like breast and nipple with protrusion. According to the statistical data, about 40% of the adolescent period male has some enlarged breast like female. This means that there are unexpectedly large numbers of the gynecomastia patients. Most of the gynecomastia is caused by the excessive distribution of the breast and fat tissue around the breast area resulting from the obesity, excessive consumption of alcoholic beverage. However, other etiologic factors like endocrinologic abnormality, tumor, and hormonal disturbance with abnormal liver function can play a role as well. Aside from the gynecomastia, abnormality of genital area and other medical problem can be accompanied, therefore these should be ruled out. Gynecomastia can be improved with the consistent diet control and regular exercise. However most of the subcutaneous fat is hard to be removed. In this case, surgical removal is optimal. Most of the cases can be handled with the small periareolar incision by ultrasound or power-assisted liposuction although excision is necessary if there is abundant breast tissue. As breast cancer can occur with this patient, tissue biopsy should be considered. Surgical treatment takes about 1 to 2 hours under the local or general anesthesia. After 1 week of convalescent period, patient can go back to daily activity. Regular exercise and diet can ensue.
