Objective : We conducted a study to evaluate prognostic value of cortisol levels and a short corticotropin stimulation test in pediatric patients with septic shock

Method : Retrosepctive review of pediatric patients with septic shock receiving short corticotropin stimulation test by IV 250mcg of tetracosactrin from January 2010 to September 2011 in pediatric intensive care unit (PICU) at Asan Medical Center; blood samples were taken immediately before the test and 30 and 60 minutes afterward. Absolute adrenal insufficiency was defined by a baseline cortisol < 18 µg/dL, and relative adrenal insufficiency by an increment

Results : 28 childrens(14 female) were included. Age, gender, PRISM score were no significant difference between survivors and nonsurvivor gruops. Absolute adrenal insufficiency was observed in 10% of nonsurvivor, 61% of survivor. (p

Conclusion : Higher basal cortisol level and smaller cortisol increase would be associated with increased mortality in critically ill children.
